Auction Permit Application

Click here for the application. This form is required to conduct an auction in the City of Neenah. No fee required. Must submit a copy of Wisconsin registration as required by Wis. Stat. 480.08.

Amusement Device Application

Click here for the application. This form is required for a license to permit the operation of a mechanical amusement device on the said premises, in accordance with the terms and provisions of Municipal Code Sec. 4-171 & 4-172 of the City of Neenah, Wisconsin.

Beverage Operator License Application

Beverage Operator License Application Packet for the application. This form is required for beverage operator license. License fee is $60 ($75 if a Provisional License is issued). Return completed application to City Clerk’s Office, 211 Walnut Street. Applicant must show a valid Wisconsin DL or other form of valid ID. Applicant must have completed Responsible Beverage Server training course within the past two years or show proof of a current, or less than two years expired, beverage operator license in a different community.  Click here for Wisconsin Dept. of Revenue approved internet Responsible Beverage Server Training courses. Licenses can be picked up or mailed after approval.

Click here to view Ordinance 2013-7, the ordinance for the Sober Server, adopted September 18, 2013. It shall be unlawful for the licensee or any employee of a licensed establishment to be under the influence of an intoxicant (with a breath or blood alcohol content of .08 or more) while performing services on the licensed premises.

Blasting Permit Application

Click here for the application. This form is required for blasting permit. There is no fee for this permit. Mail or return completed application to City Clerk’s Office, 211 Walnut Street. Certificate of Insurance must be on file with the City Clerk or must accompany application. This permit is for days specified on application only. Permit can be picked up or mailed to you upon immediate application processing.

Christmas Tree Lot Application

Click here for the application. This form is required to apply for a permit to sell Christmas trees. Permit fee is $25 and must accompany completed application. Mail or return completed application to City Clerk’s Office, 211 Walnut Street. Permit can be picked up or mailed to you upon immediate application processing.

Cigarette, Tabaco, and Vape  License Application

Click here for the application. This form is required to apply for a license to sell cigarettes, tobacco, and/or vape products. License fee is $100/year and must accompany application. Return completed application to City Clerk’s Office, 211 Walnut Street. 

Convenience Store Liquor License

Convenience Store Guidelines Class “A” Retail Liquor Licenses for convenience stores.  License fees are $250 annually with a publication fee. A special use permit is also required to receive a Class “A” Retail Liquor License.  Please see guidelines for the complete approval process.

Dance Hall Application

Click here for the application.  This license is for public dances to be held on said premise, in accordance with the terms and provisions of the Municipal Code for the City of Neenah, Wisconsin.

Escort Service License Application

Click here for the application.  License fees are $500/year, non-refundable and must accompany application.  Licenses are issued for one year beginning July 1 and ending June 30.  Return completed application to City Clerk’s Office, 211 Walnut Street.

Fireworks Sales Stand Application

Click here for the application. This form is required to apply for a permit to sell fireworks. Permit fee is $25 and must accompany completed application. Mail or return completed application to City Clerk’s Office, 211 Walnut Street. Permit can be picked up or mailed to you upon immediate application processing.

Incident Claim Form

Click here for the claim form.  This form is required to submit a claim against the City of Neenah.  For auto damages attach a copy of police report, if any.  Attach two estimates for repairs.  If you are filing both the Notice of Claim and the Claim (bottom portion of the form) be sure to include the amount of the claim and supporting documentation.  Mail or return completed form to City Clerk’s Office, 211 Walnut Street.

Mechanical Device Application

Click here for information on Mechanical Amusement Devices in the City of Neenah. A mechanical amusement device is a machine, which upon the insertion of a coin or slug operates a game, contest, or amusement, except music. A billiard table or pool table is a mechanical device when operated commercially, whether it is coin operated or not.  Video gambling machines are licensed as an amusement device.

Original Alcohol Beverage License Application

Click Here for this application (AT-106). This form allows persons to sell alcohol beverages to individual retail customers, from a particular place (premises). The sale must occur at the licensed premises, with the buyer and seller both physically present at the time of sale. Licenses are issued by the City of Neenah after the City Council determines that the applicant is qualified for the license. No one can sell alcohol beverages (or give away for a commercial purpose) or allow consumption in a public place without getting the appropriate license.  Policy 2018-1 was adopted by the Common Council on 6-8-18 requiring all liquor license applicants to complete the Plan of Operation Application.  Pol 2018-1 – City of Neenah Plan of Operation Application

Auxiliary Questionnaire Alcohol Beverage License Application

Click Here for Auxiliary Questionnaire (AT-103). Must be completed by all individuals, all partners, and all officers and directors and the agent of corporations and members or managers and agent of limited liability companies making application for a license and be attached to Form AT-106. The questionnaire must also be completed for any successor agent appointed by a corporation/limited liability company, and by all new officers and directors of corporations or new members or managers of limited liability companies applying for renewal, in which case the forms must be attached to the Form AT-115.

Outdoor Extension of Class B Premise (Beer Gardens) Application & Guidelines

Click here for the application. This form is required to serve beer/alcohol on an outdoor patio and/or serve food on the sidewalk (public right-of-way). Site plan must accompany the application and approved prior to issuance. Liquor License will be amended to have the site plan on the reverse side. Pawnbroker/Secondhand Jewelry-Article Dealer License

Adopted by reference Wis. Stats. §134.71. Pawnbroker/Secondhand Jewelry Dealers and Secondhand Article Dealers are required to obtain a permit and complete transaction records for each item purchased. To apply for one of the above licenses, file your completed, signed application form in the City Clerk’s Office. Licenses for Pawnbroker, Secondhand Jewelry Dealer and Secondhand Article Dealer for the City of Neenah are issued for a one-year, expiring December 31st. The process to obtain a Pawnbroker/Secondhand Dealer/Secondhand Article license takes approximately 3 weeks from the date of the application until the date of issuance and requires approval from the Public Services and Safety Committee and the Council.

  • Fees are as follow:
    • Pawnbroker – $210/year (Res. No. 7238)
    • Pawnbroker Bond – $500 with not less than two sureties
    • Secondhand Jewelry Dealer – $80/year (Res. No. 7238)
    • Secondhand Article Dealer – $80/year (Res. No. 7238)
  • Property Transaction Records must be kept and made available to any law enforcement officer for inspection at any reasonable time (Wis. Stats. §134.71(8)(c).

Petition for Street Improvements

Click here for the petition. This form is to file a petition for street improvements. Must acquire signatures of 50% of property owners. Mail or return completed petition to City Clerk’s Office, 211 Walnut Street for processing.

Solicitor, Peddler/Transient Merchant Permit Application

Click here for the application. This form is required for door-to-door sales or corner sales stands. First-time permit fee is $25, renewal is $15. Return completed application and fee to City Clerk’s Office, 211 Walnut Street. Applicant MUST apply in person to have picture taken to include on the permit. This application is for a one-year permit, expiring Dec. 31. Permit can be mailed or picked up after a 72 business hour hold for application processing.

Special Event Permit Application

Form required to request the use of public streets or sidewalks for special events. (i.e. parades, walkathons, rallies, festivals, etc.) A non-refundable $75 application fee may apply and must be provided at time of application submittal.  Upon approval, contact will be made with event coordinator. Taxi Cab License Application

Click here for the application. Chapter 11.07(1)(a) of the City of Neenah Municipal Code requires any person operating or driving an automobile, taxicab or other similar vehicle for hire upon the street of the city to obtain a Taxi Cab License. Chapter 11.07(1)(b) provides an exception to the code, which exempts any service holding a taxi license in another municipality from having to obtain a taxi license in the City of Neenah if they provide evidence of that license and insurance.

Temporary Class B (Picnic) License Application
