Accident Report Requests are available online via Wisconsin DOT

Citizen Commendation-Complaint Report – The Neenah Police Department encourages citizens to bring forward legitimate grievances regarding misconduct by employees. We have an obligation to investigate such allegations of misconduct to protect the integrity of the agency and its employees to instill public confidence in the agency. We also appreciate when you let us know of the good things our employees do as well.  

Gas Drive Off Form – The Neenah Police Department is committed and focused on serving and protecting our community. Incidents involving drivers pumping gas and leaving without paying are commonly referred to as “gas drive-offs.” If done intentionally, they are illegal and need to be reported to the police. When it is a simple mistake, it is a civil matter to be resolved between the station and the consumer.

Parking Ticket Citation Dispute Form – Parking enforcement is authorized by Chapter 16 of the City Ordinance. Complete an online appeal or complete this form, if you feel you were unjustly cited.

To request records, please complete the Records Request form or Mobile Friendly Request form, email npdrecords@neenahwi.gov, or call 920-886-6006. All requests are fulfilled on a first come first served basis. Responses to a request are completed as soon as practicable and without delay; however, larger, more complex requests will take longer to be completed. Requests for records of active cases will be denied pursuant to the Wis. Stat. § 19.35(1)(a) Balancing Test and Linzmeyer v. Forcey, 2002 WI 84 ¶ 31, 254 Wis. 2d 306, 646 N.W.2d 811. The law permits the Neenah Police Department to impose fees for certain “actual, necessary and direct” costs associated with responding to public records requests (Wis. Stat. § 19.35(3) . Retail Theft Reporting Packet – The Neenah Police Department encourages businesses to use this delayed reporting form when there has been a reporting delay as a result of store policy or procedure, or if the theft is discovered at a later date and the amount is under $500.00.

Ride-Along Form

Vacant Home Check Sheet – Heading out of town? The Neenah Police Department will assign a community service aide or police officer to periodically check the outside of your home to look for anything out of place or suspicious.
